eApplication

eApplications are forms for preparing and submitting electronic applications that do not require a digital certificate for electronic signatures. The form is adapted on the fly depending on the options you choose for your specific request. Once you have entered all the required information, the application will be automatically forwarded to the Health Insurance Institute via a secure channel and you will receive a notification on your screen that the application has been successfully submitted. Certain applications require scanned or photographed supporting documents to be attached to the form.
